Document Required for Accreditation for POEA
For our company to accredit the client with Philippines Overseas and Employment Administration (POEA), we need
the following documents duly VERIFIED STAMP by the Philippine Embassy / Attach� / Consulate, and authenticated by Chamber of
Commerce for PROFESSIONAL / SKILLED, as follows :

MANPOWER REQUEST/JOB ORDER � original with stamp by the company and VERIFIED by the
Philippine Embassy/Attach�/Consulate and duly authenticated by Chamber of Commerce, which is a simple letter addressed to SMC in your
letterhead stating the ff:
Category of worker required; number of worker per category; salary and other terms and condition such as overtime pay, food, ticket, etc.; and Brief job description per category. |

SPECIAL POWER OF ATTORNEY (SPA) - original with stamp of the company VERIFIED by the
Philippine Embassy/Attach�/Consulate Consulate, and duly authenticated by Chamber of Commerce. |
